    • Low Back Pain Stretch

      Today’s video shows a technique that can be used to stretch the low back and reduce tightness.

      The set up in this video is very similar to how physical therapist and chiropractors position a patient’s body when performing a low back or lumbar manipulation.

      This stretch involves pre-positioning the hip in flexion and rotating the spine by pulling on the arm. Then, pressure is applied to the shoulder and hip until a stretch is felt in the low back region.

      This video is not intended to teach you how to manipulate your partner’s spine. Spinal manipulations should only be performed by rehabilitation professionals who have been trained in these techniques. If your partner experiences pain during this stretch, stop and choose a different option.

      This talk on specific pathological disorders of the spine will be getting into the meat and potatoes of the spine. This discussion is not going to be fully engrossing, but rather an overview of the things that can go wrong.

      The topics that are covered during this lecture include: facet injury and degeneration, degenerative spondylolisthesis mechanics, spondylolisthesis canal changes, degenerative spondylolisthesis symptoms, ganglion cysts due to facet failure, lateral recess stenosis, extension narrows the lateral recess and foramen, central stenosis (narrowing of the canal and changes with flexion and extension), CT myelogram of stenosis, isthmic spondylolisthesis late findings loading and slip, spondylolisthesis with slip (late diagnosis of spondylolysis, Pars stress reactions (prior to a fracture), initial onset of Pars fractures, Pars fractures with displacement and spondylolisthesis with Pars atrophy.

      Dr. Corenman dives into a discussion regarding herniated discs with leg pain. Many patients will have back pain for months to years. They may bend forward and feel a “pop in their back” and feel jolted. For a day and half, patients are thrilled until the inflammation of the nerve comes on and their leg is on fire. This is a common scenario. Be very wary if a patient comes into your office and says, “I got a pop in my back and now they feel great.” In two days, if they aren’t complaining about leg pain, they won the lotto; however, most of them will have a leg herniation.

      There are two nerve roots at any disc level that can be affected. More topics highlighted in this lecture include: HNP locations and root involvement, herniation causing central stenosis (note flexion of spine and CT myelogram), antalgia, femoral nerve HNP (L2-4), foraminal collapse, bent knee flexed forward stance/gait, causes hyperlordosis, hyperkyphosis of the thoracic spine, osteopenic khyphosis from the loss of vertebral body height, Scheuermann’s disorder (which occurs in young kids with soft bones), sagittal alignment (flat back syndrome) and DDD, new annular tears, thru and thru annular tears, and causes of annular pain.

      Unfortunately, we are designed to have back pain which is a problem. Dr. Coreman elaborates on the beginning of degenerative changes (normal disc versus annular tear). So what happens over years and years with these degenerative changes? The disc can erode away and disappear. Dr. Coreman finishes his speech talking about Lumbar DDD (narrowing/measurement of the disc space), Isolated Disc Resorption (IDR), Modec Type I and II changes, myelopathy, Diffuse Idiopathic Skeletal Hyperostosis (DISH), arachnoiditis, hip pincer and CAM lesions, hamstring apophysitis, sacroiliac joint, metastatic disease, conus syringomyelia, and intrathecal tumors.

      With more than 30 years of experience, Donald Corenman, MD, DC, is a highly-regarded spine surgeon, considered an expert in the area of neck and back pain. His training as a Doctor of Chiropractic early in his career did not satisfy his desire for a deep understanding of the mechanics of the spine. This led him to medical school where he completed his Doctor of Medicine, Orthopedic Surgery Residency and Spine Surgery Fellowship. His study of spinal disorders and deformities brought him to Vail, Colorado where he joined The Steadman Clinic in 1994.       What are common injuries? In this video, Christopher S. Warrell, M.D., an orthopaedic surgeon specializing in sports medicine at Orlando Orthopaedic Center, discusses the common injuries he sees in his practice of medicine.

      “Hi, I’m Chris Warrell with the Orlando Orthopaedic Center as a sports medicine orthopaedic specialist,” says Dr. Warrell. “What are the most common problems I see in the clinic? By far it’s knee and shoulder injuries, and more specifically in the knee, it’s meniscus tears. In the shoulder, it’s rotator cuff tears or labrum injuries.”

      “These are injuries that are common in sports, but they’re also common in day-to-day life, whether somebody has a labor-intensive job or they have a more active lifestyle outside of work in terms of lifting weights or playing pick-up sports. Those are the common injuries we see on a day-to-day basis.”
